Medical Consent and Release Form

A current Medical Consent Form must be notarized and on file for every child participating in activities taking them off church campus. This includes all Fruits and Vines enrollees, Service Squad participants, and bell choirs. Click here for our 2010 form!

Adults serving in Children's Ministry must also have a consent form on file.
